Phase 1 : Community Outreach & Screening
Objective: To identify individuals in need and raise awareness in communities.
- Conduct door-to-door outreach in high-risk communities.
- Set up temporary outreach hubs (clinics, tents) in urban ghettos and villages.
- Offer free drug-use screenings and referrals to appropriate services.
Phase 2: Intake, Detox & Crisis Stabilization
Objective: To provide a safe, monitored detox environment.
- Conduct initial health assessments.
- Provide medically supervised detoxification services.
- Offer mental health evaluations and urgent therapy.
- Crisis stabilization and suicide prevention protocols.
Phase 3: Core Treatment Program
Objective: To address the psychological, emotional, and behavioral aspects of addiction.
- Weekly group therapy sessions focused on addiction education and relapse prevention.
- Individual counseling, focusing on trauma recovery and personal healing.
- Gender-specific support groups (esp. for women in ghettos and at-risk youth).
- Family counseling and reintegration programs.
Phase 4: Skills Development & Reintegration
Objective: To empower clients with vocational skills for post-rehabilitation life.
- Collaborate with local TVET centers to offer practical skills training.
- Provide business startup guidance and small micro-loans.
- Prepare clients for social re-entry through financial literacy and job placement programs.
Phase 5: Aftercare & Community Re-entry
Objective: To ensure long-term recovery and prevent relapse.
- Offer peer support groups for continued engagement.
- Schedule monthly check-ins for alumni and their families.
- Facilitate housing support and employment connections.
